Note Lewis is about 39 years of Age, five feet 7 or 8 inches high, a light Black, and has Scars on his right Foot, right Leg, and right Hand occasioned by Burns which he received in early Youth. It appear by the Register of free persons of colour kept in the office of the court of Hustings for the city of Richmond, that Lewis, a black man, about five feet seven inches high and about forty years old, was registered in the said office and that he deposited therein a certificate of freedom from the clerk of Staunton Corporation Court stating that the said Lewis was emancipated by the Legislature of the State of Louisiana, the 7th of April 1813 & has a scare on his right foot, right leg, and right hand.
